Mission Controlfor Your Care Ministry
Stop digging through spreadsheets and activity feeds. The Care Action Center shows you what needs attention right now — unassigned requests, overdue follow-ups, quiet risks, and care delivered this week — so you can lead proactively instead of reactively.
Four Numbers That Tell You Everything

Six Views Into What Needs Care
Each tab in the Care Action Center surfaces a different category of work. Badge counts update in real time so you always know the score.
Inbox
Unassigned care requests that need triage. These are new requests that haven't been routed to a care provider or group yet. The count tells you exactly how many people are waiting.
Risk Radar
People in your care system with no recorded contact in 90+ days. They have active assignments or open care requests, but nobody has logged an interaction. These are the people slipping through the cracks.
Waiting Room
Care requests sent to a group for first-to-accept assignment, still waiting for someone to claim them. If no one responds before the deadline, these need manual reassignment.
Overdue
Care requests that have passed their due date without being completed. These are active needs that are running behind schedule and need immediate attention.
Chaser List
Assigned care requests that are past due and have no care report filed. The provider accepted the request but hasn't documented what happened. A gentle nudge is needed.
Follow-ups
Interactions flagged as requiring follow-up. Someone visited, called, or met with a person and marked it as needing a next step. These keep the care loop from going cold.
The Spreadsheet
You maintain a spreadsheet of care needs. When someone asks “who visited Margaret?” you search through tabs. When you notice a name you haven't seen in a while, you wonder if anyone followed up. You spend Monday mornings trying to reconstruct what happened last week.
- Reactive — you find problems after they're problems
- Manual — every insight requires digging
- Invisible — nothing tells you what you're missing
The Radar
You open the Care Action Center and see 2 unassigned requests, 1 overdue follow-up, and 3 people flagged by Risk Radar. You triage the inbox, reassign the overdue request, and schedule visits for the quiet names. It takes 10 minutes and nothing falls through the cracks.
- Proactive — problems surface before they escalate
- Automatic — badge counts and tabs do the work
- Visible — you can see what you're missing
How It Fits Together
The Care Action Center sits at the heart of CareNote. Every feature feeds into it, and it surfaces the most important work across your entire ministry.
Care Requests Feed the Inbox
New requests from care requests, email forwarding, or PCO integrations appear instantly in the Inbox tab.
Activity Powers the Stats
Every interaction, note, and care report contributes to the “Care Delivered” count and clears items from the Chaser and Follow-up lists.
Risk Radar Watches Continuously
People with active care but no contact in 90+ days are automatically flagged — you don't have to remember to check.
